We are happy to notify you that the purchase of various medical & laboratory supplies under PRN-077-MED/LAB SUPPLIES-CHO-GOODS-2012, is herebyawarded to you as the Single Calculated and Responsive Bidder at a contract price equivalent to SIX HUNDRED THREE THOUSAND EIGHT HUNDRED FORTY & 00/100 PESOS (PhP603,840.00).

You are therefore required, within ten (10) days from the receipt of this Notice of Award to formally enter into a contract with us, and to submit the Performance Security in the form and the amount stipulated in the instruction to bidders. Failure to enter into the said contract or provide the Performance Security shall constitute a sufficient ground for the cancellation of this award and forfeiture of your bid security.
