You are hereby notified that the purchase of necessary and essential goods and items for the distribution of relief goods to the affected families and for the COVID-19 response of the City Government of Valenzuela (CGOV) has been awarded to your company with a total amount of  One Million Two Hundred Seventy-Six Thousand Five Hundred Pesos (?1,276,500.00).

You are therefore required to immediately sign the Purchase Order (PO) with the CGOV upon the receipt of this Notice of Award.

Please acknowledge receipt and acceptance of this notice by signing both copies in the space provided below. Keep one copy and return the other to the CGOV.
