The City Government of Valenzuela, through its Bids and Award Committee (BAC), invites the above-mentioned suppliers and all other interested suppliers, register to the City of Valenzuela and apply for eligibility and to submit the best and final offer/proposal for the hereunder project as per R. A. 9184 and its IRR Sec. 53 (1):

Approved Budget for the Contract: Php 26,500,000.00 (BC#13B; Ref. No.18-040)
Delivery Period                   : 30 calendar days from receipt of Notice to Proceed


SCHEDULE OF ACTIVITIES:
Negotiation Stage                                           : May 29, 2018;  1:00 PM
Submission of best and Final Offer                : June 5, 2018; 1:00 PM 

Please submit the following documents in a two (2) sealed envelope, properly labeled as Eligibility and Technical Requirement (original) and Financial Proposal (original) not later than 12:00 o’clock noon on June 5,2018 at the Budget Office,  2/F Executive Bldg.         

  1. SEC Registration Certificate/DTI Certificate of Registration;
  2. Valid and Current Mayor’s Permit/License;
  3. Phil-GEPS Registration Certificate;
  4. Duly signed statement of all Completed Government & Private Contracts which are SIMILAR in nature for the past year(SF-GOOD-13b) supported by documents such as CONTRACT/AWARD/COMPLETION/ACCEPTANCE, (CPES)
  5. Duly signed statement of all ON-GOING Government & Private Contracts including contracts awarded but not yet started (SF-GOOD-13a), supported by documents such as NOA, NTP, Contract, etc ;
  6. Annual Income Tax Return and Audited Financial Statement, stamped received by the BIR or its duly accredited authorized institutions, for the preceding calendar year;
  7. BIR Latest Tax Clearance;
  8. Omnibus Sworn Statement;
  9. Special Power of Attorney, for single proprietor (if the signatory is not the owner), Partnership, Joint Venture; Secretary’s Certificate for Corporation.  
  10. Any of the Following as Bid Security:
    Cash, Certified Check, Cashier’s Check, Manager’s Check, Bank Draft, or Irrevocable Letter of Credit; Bank Guarantee,Surety Bond or Bid Securing Declaration


A non-refundable fee in the amount of P 25,000.00 will be paid by the interested Suppliers to the Office of the City Treasurer from May 25, 2018- June 4, 2018, from 8:00AM to 4:00PM and on June 5, 2018, up to 10:00 AM only. Bid opening will be on June 5, 2018 at 1:00 P.M. at the 3/F Conference Room (3B), Legislative Building, Karuhatan, Valenzuela City.

The City Government of Valenzuela reserves the right to accept any bid, and to annul the bidding process and reject all Offer/Proposal at any time prior to the contract award, without thereby incurring any liability to the affected supplier/s and assume no responsibility whatsoever to compensate or indemnify for any expenses incurred in the preparation of their bids.

The Committee reserves the right to cancel or postpone the date of bidding with or without prior notice to prospective suppliers.
